COVID-19 Vaccine Clinics

The Vinton County Health Department is partnering with the Vinton County Local School District to provide Pfizer vaccines to students who are 5 to 17 years of age. Pfizer is a two series vaccine given 21 days apart. There is no charge for the vaccine and parents do not need to be present.

The vaccines will be given at your child’s school in the health clinic. If you would like for your child to receive the vaccine, please call your child’s school and request a consent and registration form. These forms will be sent home with your child. You may also pick up the forms in the school office.

Please fill out the Consent to Administer COVID-19 Vaccine and the COVID-19 Vaccine Registration Form. Your signature must appear on both forms. Please return both signed forms to the school by Monday, January 31, 2022. The Vinton County Health Department will schedule the second series and you will be notified of the date.
